Our referral process is guided by the principles of the HeartWings Model™.
This means we look beyond paperwork and focus on:
- The young person’s current needs
- Their stage of emotional development
- What environment will help them feel safe
- The type of support that will help them move forward
We only accept placements when we are confident we can genuinely meet a child’s needs.


What We’ll Ask For

To review a referral effectively, we may ask for:
- Background information about the young person
- Current living arrangements
- Presenting needs and risks
- Education and health information
- Previous placement history
- Professional reports where available

Decision & Planning
If appropriate, we agree a placement plan and arrange a supported transition.
